Extended internal medicine consultation

Thursday consultation hour with room for details, background, and individual questions — as a private and self-pay consultation. For matters that cannot be addressed in a single regular consultation slot.

Some internal medicine questions require more time than a regular consultation slot can provide — whether for detailed history-taking in complex cases, cardiovascular risk evaluation, planning of GLP-1 therapy, or nutritional medicine counseling. For these occasions, our practice offers an extended internal medicine consultation on Thursdays.

Private patients and self-payers can generally book appointments any day of the week (Mon–Fri). The extended consultation on Thursdays is independent of this — it is the right format whenever more time is needed for a topic. The two often overlap because many self-payers want exactly that depth — but they don't have to. Billing follows the official German private medical fee schedule (GOÄ).

Who is this consultation for?

  • Private patients — as a regular consultation billed according to GOÄ
  • Statutorily insured patients as self-payers — when you want in-depth counseling outside the statutory framework, e.g., planning of obesity or GLP-1 therapy, cardiovascular risk evaluation, or the sleep consultation

Typical occasions

  • Detailed obesity counseling — including planning of GLP-1 therapy (semaglutide, tirzepatide) and follow-up. See obesity therapy and GLP-1 therapy.
  • Cardiovascular risk evaluation — structured assessment of cardiovascular risk including extended laboratory parameters and carotid duplex ultrasound. See prevention.
  • Sleep consultation — detailed history-taking in suspected sleep apnea, insomnia, or complex sleep disorders. See sleep diagnostics.
  • Nutritional medicine counseling — based on internal medicine diagnostics and individual metabolic findings. See individual consultation.
  • Complex internal medicine presentations that cannot be appropriately addressed within a single regular consultation slot.

Diagnostic modules

Within the extended consultation — depending on the clinical question — different diagnostic procedures are available:

  • Duplex sonography of the carotid and pelvic vessels as well as deep vein diagnostics — see ultrasound & vessels
  • Liver elastography for assessment of steatosis and fibrosis — see liver diagnostics
  • Bioelectrical impedance analysis (BIA) for follow-up of body composition — see body composition analysis
  • Mobile polysomnography as ambulatory sleep analysis — see polysomnography
  • CGM glucose screening in suspected insulin resistance or unclear metabolic findings — see glucose sensor
  • Extended laboratory diagnostics (HOMA index, adiponectin, extended lipid profile, hormone status, etc.)

Billing and cost transparency

Billing follows the official German private medical fee schedule (GOÄ) in its current version. Scope and expected costs are discussed transparently before the service is provided. For self-pay services, we conclude a written agreement before treatment in accordance with § 18 (8) BMV-Ä.

Privately insured patients submit the invoice to their insurance as usual. Reimbursement by statutory insurance is generally not possible; some statutory funds reimburse parts of certain services in individual cases — we explicitly recommend a prior clarification with the insurance fund.

What this consultation is not

It is not a replacement for medically indicated statutory health insurance services. If a clear medical indication exists for an examination that falls within the scope of statutory health insurance, it will be performed in the regular consultation as a statutory service — regardless of insurance status or consultation format. The extended consultation is exclusively for occasions that exceed the statutory framework or where in-depth counseling beyond regular care is desired.

How do I book an appointment?

The extended consultation is bookable as a separate appointment type on Doctolib (Thursdays). In the initial consultation, we will discuss the occasion, scope, and expected costs together — you then decide calmly and without pressure whether and to what extent you wish to use the service.
